Repeat Prescribing You may order repeat prescriptions by filling out the form below. Please allow two full working days before collecting your prescription or your medicine from the Oldfield Surgery pharmacy (Hounsell and Greene).

Your Personal Details Your repeat prescription request will be sent over the internet to the Oldfield Surgery pharmacy. If you prefer that this request did not include your name and address, please enter your Patient ID (which can be found on your last prescription) and leave the name and address fields blank. This information should be taken from the tear off portion of your last prescription.
