Oldfield Surgery has a Community Nurse (Health Visiting) for Older People.
The aims of her role are:
- to promote healthy ageing and independence
- to identify emerging problems
- to prevent or reduce incidents of accidents, falls, osteoporosis, stroke
and coronary heart disease
- to promote good health
This may be achieved by providing information / advice on:
- available services and care
- factors affecting good physical and mental health
- lifestyle choices
